Have Hematological Disease. Side Effects Of Methimazole?
Question: Respected Sir,
Hello and warm greetings.
I would like your views on the attached summary of my CBC Reports of last 2 years and present year.( Attached pdf file CBC-summary )
I would like to inform you that I do not have any hematological disease . I have done so may CBC to evaluate any side effect on CBC ( especially on Neutrophiles and Platelets ) due to drug Methimazole which I am taking since last two years to treat my hyperthyroidism.
I have entered CBC – reports date wise of last two years and present year. Also make average of data.
Also estimated Mentzer Index to distinguish between any iron deficiency or disease due to thalesamia .
My present values of
Serum Iron is : 149 mg/dl
TIBC : 385 mg/dl
% Trans. Saturation : 39 %
S. Ferritin : 98
Seeking your opinion on followings ::
From the data do you find any hematological problem / disease ?
The values of MCV , MCH seems to always remain on lower side , what could be the reason ?

Thanks for your query.
All your reports are fine. Hemoglobin is maintained throughout so I would not bother about the MCV or MCH. The normal values are for 95% of the population so 5% would anyway be out of that range.
I do not find any hematologic disease in your reports. I would not get any other investigation done for this.
Please let me know if you want any other information.

However as you see in summary of reports that the average value of Mentzer Index is sliglity above 13. so may not be problem.

Thalassemia major is not possible because of the normal hemoglobin. Thalassemia carrier may be a possibility but the hemoglobin is usually bit low in that also.
This can be excluded by hemoglobin HPLC test, which is readily available.
Some people may have slightly low MCV as a normal variant. I don't think it is due to diet as then Hb, TLC, platelet should have been low (due to Vit B12 deficiency).
