To begin with there are few things to consider here. Having a stroke can cause few behavior changes in a person. In your brother’s case since it has happened twice therefore the chances are even more.
There are a few statistics that are seen in cases which can present as a cluster of the following combined or can present in isolation which are as follows:
More than 30% of stroke patients require assistance with daily living and approximately 15% require care in an assisted-living facility like a rehabilitation center
Now about as many as 33% suffer from depression or behavior changes. Which are mostly that they become more withdrawn and less social or more impulsive. So the symptoms of your brother could be that he is having behavior changes because of the stroke.
I will recommend that you should take him to a neurologist who will assess his mental state and also will look into the headaches he has been having and also will suggest the most rigorous program your brother can handle based on his age, overall health and his degree of disability from his stroke. The recommendation will also take into account your brother’s lifestyle, interests and priorities, and availability of family members or other caregivers
