Question: Homocystiene High:
I am 35 and have been diagnosed with Homocystiene of 58. The normal range told up to 12.
How serious is this? my ECG and 2D Echo was normal (I have had complete tests done ) Tests said I an diabetic 147 fasting sugar.
Rest tests were normal.

Doc prescribed me folic,b12 and other medicines. But I am worried now. How should I bring level down? Also how could it got to such hight level (58)?
I have had a stressful life and lack of routine exercise, feel giddiness sometime (-reason for these tests). Can it be cured completely? Do you have examples of people cured from such high levels? What should I do now? What precautions/tests do I need to do further?

Hello XXXXXXX

Thanks for your query.

Homocysteine is an amino acid produced in the body. Normal levels are in the range between 5 to 15 micromoles per litre and a level of 58 falls in the intermediary range. In the body homocysteine is broken down into other amino acids with the help of folic acid, vitamin B12, and vitamin B6. If you are deficient in these vitamins, homocysteine will not break down and keep on accumulating in the body causing its high levels. Other causes which can cause its raised levels are low levels of thyroid hormone, kidney disease, psoriasis or genetic causes.

The medications prescribed to you are absolutely correct and you should continue them as prescribed. In addition, eat lots of fruits and vegetables especially XXXXXXX leafy vegetables. They are a XXXXXXX source of folic acid. Other food sources of folic acid are breakfast cereals, fortified grain products, lentils, beans, asparagus and spinach. Sources of vitamin B6 and B12 are a non vegetarian diet like chicken and fish, dairy products and bananas.

You should take a good diet and these vitamins prescribed and then get your homocysteine levels checked after 8 weeks. If your homocysteine level remains high, your doctor may have you try a higher dose and get you tested for the medical conditions which can be the causes of high levels. Then you may need investigations for thyroid diseases and kidney diseases.

Avoid taking any stress as the levels should come down to normal with a proper diet and vitamin supplements. Also indulge in some mild to moderate exercises on a regular basis.

Hope this answers your query. If you have additional questions then please do post me.

Thank you doctor for your kind reply. few of concerns expressed earlier if this can be cured completely and does it mean that having high Homocysteine means sure heart problem; how can I ascertain if it has made any damage, considering my family history?

Very much thankful to you,

Hello XXXXXXX

Thanks for following up. I should have addressed these issues in my previous answer. Any way the following is my answer to you.

Honestly speaking, High Homocysteine is not as big a risk factor predisposing one to coronary artery disease as compared to other risk factors like hypertension, high cholesterol levels, cigarette smoking, diabetes and family history of coronary artery disease.

I'll suggest doing a stress ECG (tread mill test) which if negative rules out the possibility of coronary artery disease. This implies no damage is yet happened.

As far as cure is concerned, Homocysteine levels is not a disease. It is just an aminoacid which is a bit high in you. Considering you do not have thyroid hormone problems, renal or liver disease or psoriasis or genetic defect, you can keep the homocysteine levels under limits by using good vitamin XXXXXXX diet. The vitamin pills given to you will also make sure the levels are under limits.

Hope this answers your query. If you have additional questions then please do post us.

Wishing you good health.

Thanks Doctor.
I am satisfied with your answers. I had done ECG and 2D ECG at the same time when I did these tests and doctor said both ECG and 2D Echo are normal and no need to do stress ECG. Shall I still go have the tests?
