How Can A Welt Like Rash On The Lower Leg Be Treated?

Question: I've had a raised, welty rash on my left lower leg x10 weeks. It started small and spread over 3/4 of my shin. I've been treated with steroids, antihistamines and just finished 10 days of Clindamycin. It has little improvement. I've also noticed a "divet" in my shin. I'm concerned because it is not getting better.
