Regarding to tests results seems to be more than one condition that is causing your symptoms.
In my opinion, besides compression of nerves at the lumbar spine level by degerative changes of your spine and lysthesis, low vitamin B12, D, together with kidneys dysfunction, and moderately high blood glucose and uric acid levels, are contributing to your symptoms.
So, as a first step, it is necessary to correct these findings by using NSAID drugs and physical therapy for the degerative spine disease, vitamin D and B12 supplements, treatment of high glucose levels, treatment about kidneys dysfunction.
Other abnormal findings such high cholesterol, etc. should be addressed too.
