Brief Answer:
Clinical examination is helpful.

Detailed Answer:
Hi,

Thank you for posting your query.

I have noted your dilemma and it can be resolved by a good clinical neurological examination.

In PSP (progressive supranuclear palsy), there is more axial rigidity (involving neck and trunk), head and neck are held in extension (backwards), there are usually no or less tremors of hands, there is restriction of eye movements (up and down and sideways) and backward falls.

In contrast, in PD (Parkinson's disease), there is more rigidity in arms and legs; head and neck are held in flexion (bent forwards), there are tremors in hands; eye movements are normal; and there are forward falls, if any.

If there is doubt after clinical examination, PET scan of brain can be done to sort out the diagnosis.

I hope my reply has helped you.

I would be pleased to answer, if you have any follow up queries or if you require any further information.
