Brief Answer:
Continue the course of medications for 21 days.

Detailed Answer:
Hello Mr XXXXXXX
Welcome to HCM,

I have gone through your query and appreciate your concern for your wife.

In my opinion the symptoms described by you fit the diagnosis of secondary amenorrhoea.

Combined hormonal oral contraceptive pills are used for 2-3 cycles for irregular period to make the hormonal axis stable.

The failure occurs due to different causes like, missing the pills or premature menopause etc.,

You have mentioned that your wife missed 3 pills, that may be the cause of the failure of the treatment in reestablishing the menstrual periods.

I would advise her to take the pill for another 1 cycle and then wait for menstruation.

Even after continuing the medications for another cycle if your wife does not get her periods, then you will need to get her hormonal profiles evaluated to look for any abnormality in them.

Please do write back to me with the following details so that I can guide you further.

1. During these 5 years of not getting periods, was your wife on any contraceptive medicines?
2. What was the mode of delivery of the first child?
3. Have you got any tests done, if yes, please upload all the reports for my review.
4. How were your wife's periods before she became pregnant first time?
5. What is the profession of your wife? Does her work involve continuous stress?
6. Do you plan to conceive again?

Please do write back to me with the requested details so that I can help you better.

I hope you find this helpful.

Awaiting your response.

Regards.

Brief Answer:
Hormone Replacement Therapy might be helpful.

Detailed Answer:
Hello,
Thank you for the informations you have provided Mr. XXXXXXX
I have gone through it.
You told that your wife is having early menopause as told by her doctor.
Early menopause is one of the common causes of Secondary Amenorrhoea.
Premature ovarian failure is the most common cause of Early menopause.
The causative factors of Premature ovarian failure are commonly:
Autoimmune diseases
Tuberculosis of the genital tract
Smoking
Radiation and/or chemotherapy
Ovarian failure following hysterectomy
Prolonged GnRH (Gonadotrophin Releasing Hormone) therapy
Enzyme defects
Resistant ovary
The most specific diagnostic feature of this is high level of FSH and low Estrogen.

The symptoms of Early Menopause are more or less same as normal menopause..
Like--
Progressive atrophy of genital organs,
Vaginal dryness,
Decrease in Libido,
Osteoporosis,
Psychological changes etc.

Now about the treatment;
In case of ideopathic premature ovarian failure it is not reversible.
Only method of treatment is to decrease the symptoms of Menopause.
This is done by hormone replacement therapy,
Selective Estrogen Receptor Modulators,
Antidepressants
Lifestyle modifications etc.
you told that you don't want another baby. But if you want that is possible by donor's ovum and husband's sperm.
you should consult her doctor too.
I hope this will be helpful.
If you do not have any clarifications, you can close the discussion and rate the answer. Wish her a good health.
Regards.
