Fungal infections can sometimes be difficult to treat because fungi respond slowly to treatment.

It can sometimes take 2 to 3 weeks for skin infections to resolve and if a fungal infection involves the scalp, toenails or fingernails, it can take longer.
The cream would have to be used consistently till the area starts to improve.

If the itching and rash is spreading then you may need to add oral anti fungals to the topical treatment that you are using. This may in the form of fluconazole, lamisil or sporanox.

You can speak to your doctor about this option.
