How Long Should Montair Be Given For Wheezing Cough In An Infant?
Question: my two and half year old son, who had wheezing cough, has been detected with WALRI, and has been prescribed to nebulise with 0.63 mg Livolin at 4 Hours interwal, omnacortil for 5 days and Montair 4 for next 4 months. Is it correct to give montair for so long period. Please advice.

Most of the wheezing cases are either due to viral infection or allergy.Short course of oral steroid like omnacortil with nebulisation with some bronchodilator like levolin helps in recovery.
Montair is an long acting anti allergic and is usually given to the babies who have past history of similar episodes.
To prevent the recurrence it can be given for 2-6 months.The side effects are very minimal if any even after prolonged use.
If there is no significant past history it is usually given for 10-15 days.

Yes my son has past history of recurrence of this allergy. During earlier/ last recurrence he did no had fever, but this time he having fever also, temperature hovering around 100- 101 degree(For fever doctor has prescribed Crocin DS - 3.5 ml), but there is no running nose. Is it safe to give omnacortil during fever?

In most of the babies the wheezing episode is precipitated either by allergy or some viral infection.
But bacterial infection can also precipitate the wheezing episode.Fever can be there in both viral and bacterial infection,differentiation is usually done with blood test (c reactive protein and hemogram).
Steroid if given in bacterial infection can further flare up the infection.
So I don't recommend to give steroid if bacterial infection has not been ruled out.
