How Long Should Pantoprazole And Melox Be Continued?

Question: Pantoprazole and melox are given to my dad based on previous advice. Diarrhea is under control now. How long should he use those medicines? We are giving him 1 tab (Lowest dosage) for pantoprazole and 8ml of malox once a day
Please look at my previous question as well for context. I also would like to know what can be done for his weakness? He can hardly take 20 steps and gets tired very badly.

Hi,

I have gone through the medical history of your dad and understood your concerns.

1. Pantoprazole is a ppi medication used to treat stomach ulcers or gastritis. He can use up to 8 weeks.
Maloox is an antiacid and can be used for 2 consecutive weeks. After that can use each time he might have problems (burning, pain etc)

2. With regards to being weak, I was reviewing his other data and noted that he has a previous history of gi bleeding.

As blood has been lost, your dad might have become anemic. Anemia itself is characterised by weakness as major and notable symptom.

At this point, I recommend to run a complete blood count to see if anemia is present. If confirmed, then appropriate treatment is suggested.

Taking into account all his medical history, I also recommend to repeat upper endoscopy for a reevaluation.

attached is CBP result. in addition, my dad is getting his diaper changed even though he did not go for motion. I suspect it to be gas. he does this at least 2-3 times a day. please advise.

I read carefully your blood work and I can say that your father has moderate- severe anemia with low hemoglobin and low RBC account.

So at this situation I strongly suggest to contact immediately with your doctor or you should go immediately to ER in order to exclude gastrointestinal bleedings.

At our previous conversation your father has experienced GI hemorrhage like melena. This explains his CBP finding and his symptoms like weakness.

Currently he is not having any GI Hemorrhage or has any stomach upset. I would like to know what can be done to improve his RBC as well as haemoglobin.

Will occult test help in ruling out any GI Hemorrhage at the moment? If yes, I can get it done to rule that out for sure.

I would also like to know what can be done with regards to the Gas? At times he is thinking that he passed the motion but all he passes is gas.

Hi back,

First of all I would like to suggest doing ocult blood stool test. This will help to exclude any minimal gastrointestinal bleedings.

Secondly you should start him treatment for anemia including:
- iron tablets
- folic acid and vitamin C
- eat foods reach in iron and vitamins such as spleen, liver, salads, spinach etc.
This will help to raise his RBC, hemoglobin and other blood parameters.

Thirdly I would recommend to use simeticone to decrease the gas and to relieve his symptoms.
