Question: My wife is 14 week pregnant, she is diabetic now and taking 08 unit of insulin but pp glucose level after meal ranges are coming on avg of 155-175. what to do lower glucose level. Other medicine like eco spirin 75 and duphaston and thyronorm for thyroid 150mcg she is taking,

Hello,
gestational diabetes mellitus has to be quantified first if its gestation induced or yu have been diabetic before pregnancy.
Firstly, get a HbA1c levels done , if value more than 6.5% then you have been diabetic before pregnancy.
but if less than 6% but OGTT with 75gm glucose F=92, 1HR=180, 2HR=153 mg/dl then if any one value is high then she is gestational diabetes patient.
GDM can be controlled by following a strict protocol of diet with 3 major meals divided into 2/3rd and 1/3 rd = 6 small meals and 3 minor meals as n snacks daily after calculating the total calorie intake required as per her BMI.
Regular blood sugar 6 point profile is required to be done 3 weekly to establish control. I

If after diet control, still few values are raised then glibenclamide can be added , and if still uncontrolled then only insulin should be resorted to.
I feel switching directly to insulin is not the right thing to do. Please discuss with your physician regarding specific GDM MANAGEMENT FROM obstetrics point of view and not from endocrinology point of view at all.
