How To Detect Premature Ejaculation Problem? Treatment?
Question: Hi, how do i know i hv a premature ejaculation problem? And if i hv tht issue. How to treat it? I am getting married after 6 months, how should i prepare myself on sexual health for that?
Hello and welcome to Healthcare Magic.
Premature ejaculation is a condition where a man ejaculates too early. You can know if you have premature ejaculation if:
1) ejaculation occurs very with minimal penile stimulation.
2) when ejaculation repeatedly occurs before satisfying the partner.
3) generally, a cut off time of less than 1 minute is considered as premature.
There are several ways to treat this problem of premature ejaculation. There are medication which delay ejaculation and there are sexual counselling methods.
Regarding HIV, the only way to know if you have it or not is to do a HIV ELLISA blood test.

Hello again,
There are certain specific sexual techniques / penile exercises which can help in delaying ejaculation. The basic principle of these exercises is to reduce penile 'hypersensitivity'. A hypersensitive response of the penis is a major cause for premature ejaculation, especially in newly married men.
The "squeeze technique" and the "stop-start technique" have been proven to be successful in reducing the 'hypersensitivity' of the penis and thereby increase the ejaculatory time.
In the squeeze technique, the penis can be stimulated by hand (by masturbation) till a point when you feel close to orgasm. At that point, the lower part of the head of the penis is squeezed tightly with the index finger and thumb around for a few seconds till the urge for ejaculation stops and the erection decreases. Then the stimulation is restarted and the process is repeated a few times.
In the stop-start technique, the penis is stimulated by masturbation till a point when you feel close to orgasm. At that point, the stimulation is stopped for some time and after a small gap, it is continued. This process is repeated repeated a few times.
Both these penile exercises, if practised regularly, can gradually increase the time taken for ejaculation.
There are also certain medication like Dapoxetine (Prilyxet), which can help you with this specific problem of premature ejaculation. But I would advise you to keep medication as a last option only after trying the penile exercises and only if the problem is severe.

Thanks Doctor! One last question, what is sexual counselling that you mentioned about?
Sometimes, anxiety / fears / doubts about inadequate sexual performance - called as "performance anxiety" can be one of the important causes for sexual problems. Also, misconceptions or poor understanding about sexual health and functioning can lead to sexual problems.
In fact, many young males have such doubts, fears and anxiety about marriage and sexual performance. In such situations, sexual counselling can help. This approach is both an educative approach as well as a psychological approach - to help the person get rid of fears that he may be sexually inadequate or doubts that he may not be able to perform properly in bed. It also helps the person improve communication and intimacy with his partner and build confidence in the marital relationship. Sexual counselling has to be done by a qualified psychiatrist who specializes in sexology.
