Question: Hi iam a warfarin patient and trying to conceive, I had a miscarriage in March this year after nearly 4years of trying to conceive, iam really desperate to get pregnant again what can I do now to help increase my chances and get pregnant quickly?

Do keep a good observation on your bleeding time and clotting time regularly by getting the same tested in a lab.

You can try for ovulation induction with medication like Tab Clomid. This increases the chances of ovulation and also provides a idea on ovulation timings, which helps to plan the intercourse and hence the pregnancy. The chances of pregnancy with clomid are very high and planned too.

I have been on clomid already as well, I have been ovulating according to the ovulation kits, Also I read that warfarin interferes with the progesterone hormone so I would like to know if this is the case. We have been trying for nearly 4 years now and then when I did get pregnant at the beginning of the year I miscarried, we got a heartbeat and then it was as if 5 days later the pregnancy failed. According to the doctor it was just a failed pregnancy and a possible chromosomal problem with the baby. I want to know if it is necessary to get extra progesterone tablets to help increase the chances of holding and getting pregnant? Or is clomid a progesterone based tablet?

Hi,

Thanks for writing back.

Warfarin does interact with progesterone, the effect of the same is in widening of the side effect profile of the warfarin. There occur numerous complications while carrying but not reduce the chances of conception. Some of the side effects include Premature Labour, Placental Insufficiency, Foetal Growth Restriction, Premature Baby etc. Hence, once you are pregnant it is wise to withdraw the warfarin and place a substitute.

The loss of your previous pregnancy may not be associated with warfarin as it was proved that it was due to chromosomal defect.

Taking extra progesterone is not needed, as there is no depletion of progestrone due to warfarin.

Clomid is not a progesterone based medication. It simply increases the chances of ovulation.

You may have to get investigated in detail for the delay in conception. Hypothyroidism may have to be ruled out as it is one of the common complication due to warfarin.
