I can see that there is post-inflammatory hyperpigmentation (PIH) along with lichenification of skin. In recurrent fungal infections due to constant scratching an element of eczema sets in and the skin becomes lichenified (dark and thickened) which itself is an itchy condition and thus it becomes a cycle, the more you scratch the more it itches and the more is the desire to scratch (Itch Scratch Itch Cycle). This cycle is perpetual if the fungal infection is recurrent.
The best way to deal with a situation like this is to treat existing fungal infection and prevent recurrence. You may use a mild-moderately potent topical steroid preferably in combination with an antifungal (e.g Clotrin B cream) twice daily, the antifungal would take care of the fungus and the topical steroid would take care of the lichenified skin. Along with this I would suggest weekly oral fluconazole 150 mg for 4-6 weeks.
Regular use of an antifungal dusting powder in the groin folds even after the condition abates is advisable to prevent the recurrence of fungal infection because it not only keeps the folds dry but also being antifungal exerts inhibitory affect on fungus growth.
An OTC antihistamine e.g cetrizine 10 mg once daily would provide you symptomatic relief from itching.
