Question: I am on dialysis since last two years.

Aten 50
Nicardia CD 30
Arkamin.
During the initial phase of dialysis my BP tends to be normal... Later as it progresses and reaches the end...it goes up to 170/110.

Brief Answer:
Its usual.

Detailed Answer:
Hi

Greetings from Dr. Divakara.P

It's very usual for the blood pressure to go high during or after dialysis. What is important is for how long your BP remains on that day.Kindly let me know that.

Also let me know whether you take your Blood pressure medications on the day of dialysis or you won't as most of patients on dialysis would have been advised to so.

Will be looking forward for your reply.

The BP remains high throughout the day.
I also take medication like nicardia and arkamin 30 ?mins before dialysis.

I also take the other medicines on time twice daily

Brief Answer:
Need to know the exact total daily dose of each medicine

Detailed Answer:
HI

Thanks for the follow up .

You said you take medicines twice a day. So your total daily doses are

Prazopress 10mg , Aten 50mg , Nicardia CD 60 mg right ? Just make it clear for me .

Also what is the total daily dose of Arkamin?

See the basic principle is if you have not reached the maximum doses of tablets then we can increase their dose instead of adding Targit . But if you have reached the maximum doses of tablets then we have to add Targit for your exiting medicines .

Kindly get back to me .

I take prazopress 5 mg twice a day
Aten 50 twice a day
Arkamin twice a day
Nicardia CD 30 twice a day.
Omnacortil 5 mg once a day
Lasix 40 twice a day (morning and afternoon)

I have undergone one kidney transplant in 2003.
That third kidney has been rejected in 2013. It was unrelated transplant.

Since then on dialysis.

Arkamin is twice daily 100 MCG each. so total 200 mcg

Brief Answer:
Increase Arkamin dose

Detailed Answer:
HI

My recommendation to you is to increase the dose of Arkamin to 100mcg 3 times a day .

Keep checking your Blood Pressure every alternate day. If it is still high after 2 weeks the dose of Arkamin can be increased to 100mcg 4 times a day ( that's the maximum dose ). Even in that case Blood pressure is not coming to control revert back to me .

Hope this information was useful to you.
