Is CABG The Best Treatment Option For Ischemic Heart Disease?

Question: Dear Doctor,
This is with regards to Medical Condition of My Uncle, 61 Years Old, who has been suggested CABG. I want to know whether the same is very much required and within what time frame it has to be done. I have attached 3 relevant medical reports and awaiting your feedback.

I read your query and understood your concern.

Based on the data you provided, your uncle suffer from ischemic heart disease (three vessels disease or 3VD in his reports) complicated with low ejection fraction (EF 39%) and heart failure. This is why the best thing to do in your uncle case is triple CABG.

The best time frame to do CABG is within two weeks from now. This time is needed to stop any blood thinner he is already taking (deplatt A) before surgery.

Many Many thanks for your reply.

I just want to know one more thing that whether his reports is showing any VALVE LEAKAGE (REGURGITATION), if yes, then what is the treatment applicable for the same.

(1) Whether it will be treated on the operation table during bypass.
(2) If it can be recovered by medicines.
(3) What can be a later complication due to the same.

Thank you for your follow up queries.

1. I have gone through the echocardiogram report you sent in, and I can say that it is noted a mild mitral, tricuspid and pulmonary valve regurgitation together with atherosclerosis of aorta with no presence of valvular stenosis (narrowing).

Based on the echo-reports, my opinion is not to run surgery although the final conclusions are finalized during operation.

2. Usually, in such a case, it is advised to keep taking medications as long as echocardiogram reports will show no indication for surgery.

3. In such cases, no long term complications can be foreseen. However, I can say that surgery is indicated only in emergency cases of short-term complications.
