Is Clonidine Effective In Treatment Of Anxiety?

Question: I have been hearing a lot about this clonidine, not just for high bp, but for anxiety. I have been on benzos for about 25 years now and of course I have built a tolerance to where My dose is ineffective and my doc will not raise it. I also heard of possible help with tourettes. I would like to try it, if [ and probably not ] my doc will give it to me. My question is it safe to take being as I am taking these 2 bp meds already? I also heard it is good for smoking cessation .

Clonidine is a drug of alpha 2 agonist receptors class. The drug is used in a number of disorders like you have mentioned hypertension, Tourette's syndrome and also in opioid withdrawal (to decrease symptoms of withdrawal and anxiety), ADHD etc disorders. It is approved for use in anxiety associated with ADHD in children.

As the drug result in decrease in autonomic symptoms and also improves hyperarousal so it is also used in conditions associated with such changes like opioid withdrawal, in anxieties like in post traumatic stress disorder, following withdrawal from benzodiazepines to reduce the symptoms.

You are already taking Losartan/Hydrochlorthiazide (100/25) and Propranolol (60) per day so theoretically there is risk of decrease in heartbeats (bradycardia) if clonidine is used. But the risk is not so high. Over all if clonidine and propranolol are used together they may prove useful in maintaining BP.

There is risk of sudden hypertensive crisis if clonidine is stopped abruptly. Clonidine may antagonise some effects of propranolol but in practical the effect is not seen.

Secondly clonidine is usually prescribed in evening time as it causes sedation while propranolol is given in morning hours so there will be less risk if used in that manner. The drug will help in good sleep and will decrease anxiety also.

You can talk with your doctor and can discuss for clonidine in low doses like 0.1 mg per day. The doses will reduce anxiety and will not cause significant hemodynamic instability.

Similar effect is seen in smoking cessation as it reduces autonomic arousal like giddiness, restlessness, palpitations, sweating etc symptoms due to nicotine withdrawal.
