Question: HI my wife is 8 week pregnant and her TSH reading is 4.78 doctor prescribed ebexid is it safe to take this tablet during pregnancy??

Thyroid IP 85................................. 15 mg
Nicotinamide IP ........................... 25 mg
Pyridoxine Hydrochloride IP........... 1 mg

The value of TSH in second trimester should be less than 3.5 and in third trimester, it should be less than 4.
Ebexid is safe in pregnancy, but Thyronorm or Eltroxin is a better drug.
Better to start 25 microgram Thyronorm daily on empty stomach, and then repeat the test after 6 weeks to see the response to treatment.
Yes, in pregnancy, the TSH level increases.
Your wife's value is very low, not very abnormal, so it would mostly come down to normal in future after pregnancy.
