Question: Your evaluation of TSH of 3.8, T3,Free 3.0, T4,Free 1.21?Would synthroid 100 MCG be indicated?

Hello and thank you for asking!
From the result you reported the Ft3 is slightly decreased.
The TSH is within the normal ranges.
If you do not have symptoms of hypothyroidism (feeling sleepy, constipation, low body temperature or high cholesterol level) I would not recommend to start levothyroxine.
I would suggest also to try to find the cause of under active thyroid in order to decide the frequency of follow up.

I have been treated for hypothyroidism for the past 12 years. About 2 years ago, my Dr. noticed my TSH climbing and upped my Synthroid from 50 mcg to 75 mcg, as I was gaining weight and feeling tired all the time. I do have hypercholesterol and take Vytorin. A recent recheck of labs shows increased TSH as reported above and therefore it was suggested to increase my Synthroid again.

Now the situation is much clear.
If the hormonal result you reported is under treatment, it means that the actual dose is correct.
The TSH level is the indicator of the dose of the last 2 months. At your age is suggested to keep it at the upper normal ranges.
So 3.8 is the recommended TSH level you should keep.
The symptoms of hypothyroidism are very discrete and the TSH level is the only reliable indicator of the levothyroxine dose.
