Question: Sir , i am 40 years old. twice cesarean operation was done . Right now I have bulky fibroid uterus,multiple hypoechoic lesions largest measuring 5.2 cm, uterus size 16.2 x 8.2 x 9 cm, left adnexa shows cystic lesion measuring 5.2 x 4.9 cm, Endometrial thickness 6mm, No right adnexal mass lesions. No free fluid in POD. No ASCITes/ Pleura l effussion
non surgical treatment/ surgical, am afraid of third operation at this 40 age

Fibroids do not need treatment unless symptomatic, however this big a fibroid ( 5 cm with left adnexal cyst) does require treatment. Generally this size of fibroids does not respond to conventional medical treatments, hence surgery is a must.

The mainstay of treatment of symptomatic fibroids has been surgery, which could be either myomectomy (removal of the fibroid), or hysterectomy (removal of the uterus). Of these, only hysterectomy ensures that the patient will never suffer from fibroids again. Both hysterectomy as well as myomectomy can be done laparoscopically (key-hole surgery), dramatically reducing the post- operative morbidity. However, even these are invasive techniques, and the risks and possible complications of surgery and anesthesia remain a constant threat.
After two C- sections, hysterectomy can be done and it will give you a permanent relief.

Is the key hole surgery done at any hospital are particular and cost ? Is it good to remove the fibroid and take chance to post pone hysterectomy? What are the side effects if hysterectomy is done? IS it immediate or how long we shall wait for surgery? I am 5.7ft. weight 88. having BP tablets for the last 8years. Thyroxin for the last 1 year, No Sugar . No more complications in connection with health. Will be there any problem to work after the surgery ?

Thanks for writing again.
The key ho;e surgery is done at various hospitals. Your gynecologist can guide you regarding the one best for you.
Only myomectomy will not help in your case as you have an ovarian cyst also that also needs to be removed. Also you have multiple fibroids that are difficult to be removed by myomectomy.
Hysterectomy is a major surgery and is associated with all complications of a major surgery like infection, fever, delayed recovery, etc. With proper post operative care, these complications can be minimized.
The time for which you can wait depends on the rate of growth of cyst and fibroids and the severity of symptoms. This can be assessed by your gynecologist after an examination and repeat investigations after 6 weeks.
You cam do your daily work normally almost 6 weeks after the surgery.

laparoscopic vaginal hysterectomy / abdominal laparoscopic hysterectomy which is better. surgery date has any relation with dates/ pre- operation diet. any symptoms of cancer/ Is any other organ damaged by the fibroid and ovarian cyst/

Vaginal hysterectomy is a better option. You can have a healthy diet before surgery that will help you in earlier recovery. Surgery date depends on your surgeon. Cancer is not likely. Histopathology report after the surgical removal will help in ruling out cancer completely. Large ovarian cysts can be associated with ovarian cancer occasionally.
There is non added problem due to age. Detailed physical check up is needed to rule out any other diseases.
At this age, rate of surgical removal of uterus is 5-7%.
