Is It Normal To Have Spotting And Irregular Menstrual Cycles During Perimenopausal State?

Posted on Fri, 4 Sep 2015
Question: hello, this is my second question, first was answered by dr XXXXXXX I am 46 years and diagnosed in perimenopausal stage by MD Gynaec. I had my cycle on 26th XXXXXXX after which I had spotting on 7 july I was administered deviry for 13 days and on 12th day of deviry light spotting started and after 13 deviry immediately the next day menses started. which was on 23rd of july. since 2nd of august I was asked to take cycloreg for fifteen days. on 9th night I had light spotting which stopped by next morning. I was asked by doc to continue cycloreg during spotting too. again on 9th august there was very light dark brown spotting. ( just slight staining on underwear ) I again talked to doctor who said to continue cycloreg and if spotting increases we will change tablets.....yesterday night I had abdominal cramp which I get during or before menses and today morning had same dark brown spotting just once......does this happen while on cycloreg....is there anything to worry. my usg is normal, thyroid test, sugar is normal, intra uterine usg is normal.....there is 36mm clear water cyst in left ovary which doc said is nothing to worry about. CA 125 was conducted to rule out endometriosis and the result was 36 against the reference result of 35 which the doc said is nothing to worry about.....can you please explain if its okay to have very light staining or spotting which doest require to take pad during taking cycloreg. I have to take it till 16th august. also does 36 range in CA125 is something to worry about? is abdominal cramp during perimenopausal stage common?

Brief Answer:
Nothing to worry.

Detailed Answer:
Hello
Thanks for writing to us with your health concern.
First of all I wish to know how were you diagnosed as peri menopausal ?
If the ultrasound scans and blood tests are normal, again there is nothing to worry.
There was no need to start Deviry also, as mild spotting and irregular cycles are common during perimenopause.
Can you upload the reports / papers where the diagnosis of peri menopause has been made for you ?
Also, yes it is very common to have slight spotting or brownish bleeding while on Cycloreg, so please do not worry.
CA 125 range is perfectly alright, again nothing to worry.
Cramps may occur while on Cycloreg, again that is nothing major to worry about that.

Brief Answer:
Can confirm perimenopause with objective tests.

Detailed Answer:
Okay, but the diagnosis can be confirmed by hormonal tests - FSH, LH, Serum estradiol.
These levels will clearly tell you where you stand with respect to menopause.
THat sensation is also because of peri menopause and the progesterone that you are taking.
You can take mild antispasmodics if the cramps are severe, else just let it be.
It will pass in a few days.
Take care.
Exercising regularly and a healthy diet plus meditation / yoga will also help you a lot.

Brief Answer:
It is a side effect of progesterone therapy

Detailed Answer:
Progesterone has that side effect.
It is typical of progesterone therapy.
Plus you are just recently using it, it will take a few months for your body to adjust to the hormone.
Take care.
