Is It Safe To Switch From Morphine To Opana?
Question: my DR. has switched my Degenerative disc Disease (diagnosed 20 years ago) chronic pain RX from 60 mlg. per day of morphine er to Opana er but only 10 mlg per day-5 mlg, every 12 hours.... I was told these two meds are equivalent. I was stable on my previous dose for many years, began with 1 15 mlg. morphine
20 years ago and gradually to 60 mlg. for last 6 months. I live in a state where even terminal cancer patients seem to get push back from their PC physician. I do not have adequate knowledge of the chemical composition of these two different meds to make an intelligent arguement. Please help as I am suffering but do like my DR. and have been with him about 4 years. THANKS,IN ADVANCE

Hello
Thanks for posting
I have gone through your query and understand your concern. The opana ER contains semi synthetic analogue of morphine known as Oxymorphone, hence both are nearly equivalent.
It takes a few days to week time to adjust, so just follow your doctor advice. It should be taken at the same time of day either 1 hour before meal or 2 hour after meal. You may add Ibuprofen in between to relieve pain after discussion with your doctor.
Hope this answer your query and I shall be glad to answer further.
Take care

Thanks for your answer. My PC physician told me he utilized a conversion chart for medications to determine the Opana(oxymorphone) equivalent to 60 mlgs of morphine sulfate ER. This is my primary question because I've been on the Opana now for over a month and my pain has never fallen lower that a 7,(based on the standard pain scale of 1 thru 10). When I was on the morphine,my pain was better controlled and consistently registered a 3 or 4 on the scale. This is an issue of quality of life, since my degenerative disc disease,by definition, will only get worse,I'm desperate to maintain a daily routine of normal activity ie. light housework, hygiene, walking etc. With only 10 mlgs. of opana per day, this last month has been a poor excuse for living as I mainly lay in bed on a heating pad...I can't even manage grocery shopping. Is there access to this RX conversion chart on line? I see my Dr. on Nov. 8th and hope to convince him to put me back on the morphine-I do not believe that 10 mlgs. of opana is equal to 60 mlgs. of morphine.....my body tells me so. This is my follow up question to any Dr. in this platform that can help me. THANK YOU!

Hello
It's good to see you.
I have gone through your reply carefully and have full sympathy with you. We usually start with low dose and if require may increase the dose further. This is to prevent the side effect.
The conversion table for morphine to opana ER is 0.33.Hence; if you are taking morphine 60mg the opana ER should be 20 mg/day that is 10mg in morning and 10 mg in the evening. You may see for yourself by logging at http://www.opana.com/hcp/about-opana/opioid-conversions.html
Since, your life is totally disturbed by 10 mg/day, you may discuss with your doctor to increase it to 20mg/day.
Wishing you good health.
Regards
