Unfortunately, the chances of natural pregnancy are almost nil since she is 41, and as per the reports, there are some problems with sperm shape and morphology. Percentage of normal sperms is very low.
Please consult an IVF (in vitro fertilization) specialist at the earliest, and go for ICSI - intracytoplasmic sperm injection, provided the tests show that your wife is having the capacity to produce a good number of eggs.
