
Is Retina Vitrectomy An Effective Treatment For Retina Detachment?

i have had retina detachment in the eye 4 weeks back. did retina vitrectomy.
3 weeks later the surgery doctor told me that the healing is perfect and asked me to come after 2 months to remove silicon oil. since yday i felt like lost some of the oil in my eye and went to doctor today for a consultation. he saw and asked me to come after a week and said he see some oil got lost and there is slight diplacement / fold in retina.
he asked me to use vegamox and tears gel for every 2 hours for next one weeka nd asked me to come next tuesday. depending on the need either he may do procedure again and inject silicon oil or just go on drops.
he okayed to resume my duty and even okayed for air travel.
i am worried of this complication and pls guide and advise me if anything to worry.

I reviewed your history. It seems from the history that you have undergone surgery for retinal detachment in the form of vitrectomy with silicone oil insertion.
No need to worry as this problem of retinal dislocation is common and is very common in complex retinal detachments and following retinal detachment following perflurocarbon injections. Though not very common following silicone oil injection it is seen in quiet a number of cases.
Not all cases require further manipulation. Only the cases with involvement of the central part of the retina called as macula may require further treatment.
Hence no need to worry about this problem you will be fine. Only if your vision is affected then further course of action is required.
I do follow up many patients with this problem following retinal detachment surgery and only very few may require repeat surgery.
Hence I advice you not to worry and continue the treatment as advised by your doctor.

I reviewed the reports attached with the question.
The need for further silicone oil injection depends on the site of dislocation of retina and the visual acuity.
As of now I don not think there is a need for repeat injection of silicone oil. However wait for the next visit and the report of the eye examination and depending on it it can be decided. But most of the cases does not require repeat injection in my practice and clinical experience. Hence do not worry much about it.
You can plan air travel, no issues. The precautions that need to be taken or to avoid any injury to the affected eye, do not squeeze the eye or strain the eye.

one last query . where the oil would have gone? outside or inside. if its gone how only partially leaked ? and not all gone.
pls answer this last query.
It means it is not providing enough tamponade.

Thank you for the follow up question.
The silicone oil would have not gone anywhere but the thing is in some areas it will not provide tamponade thus causing the retina to dislocate.
Only in long standing cases the silicon oil will emulsify and reach the anterior chamber of the eye.
Hence I advice you not to worry much about it, it is a part of surgery to have this problem and I hope that it will be fine.
