Is Safe To Give Moxclav For Chest Congestion In An Infant?

Question: Dear Dr,

My 2 1/2 month baby is suffering from congestion in the chest and has been vomitting phoelm with milk. Should I be giving her Moxclav Bd as prescribed by the doc for 5 days 0.5 twice a day. Isn't the baby too young for Antibiotics?

Hi....many a time, cough and cold and chest congestion are of viral origin in kids. Infact almost 95% of times its viral. No need of antibiotics.

I agree with you, an antibiotic like Moxclav is not indicated without proper evaluation for a kid less than 3 months old.

To be XXXXXXX leave alone antibiotics, even antiallergics are not approved by FDA at this tender age. If it's a viral cold it will get better in 5-7 days. The chest congestion you quote, I assume us the sound the baby makes while breathing and it could as well be stuffy nose too.

I suggest you stop the antibiotic, start giving only saline nasal drops like Nasivion-s, 1 drop in each nostril every 4-6th hourly, that too only if there blocked nose.

Dear Dr,

Thank you for your reply. Further to your answer how do I avoid the child vomitting milk and phlegm.

Also, she howls while having milk with diya as I donot have enough breast milk. How can I start her bottle?

1. Its common of kids with cough and cold to cough and vomit due to that.... That's how the phlegm comes out. So do not worry.

I have a few questions before advising top feeding.

1. How much time does she feed at your breast?
2. What is her birth weight and current weight?
3. What is her exact date of birth?
4. When she's on only your milk, how many times does she urinate in 24 hours?
5. Do you feel fullness in one breast while feeding in another?
6. Will the milk trickle in another breast while you are feeding in one?

Kindly get back with answers so that I can help you scientifically.

Dear Dr,

My baby was born on 18th August. Her birth weight was 3.1 kg. Though she latched very well initially and was exclusively on my feed, she developed jaundice and her billirubin was 25 and underwent phototherapy for 5 days. In this period, she was given a combination of top feed and expressed milk. I am o+ and she is RH -. I underwent a surgery and hence not feeding her as her weight wasn't going up. As on 15 oct her weight was 4.260 kg and on 22 oct it was 4.395 kg. On 1 nov she weighed 4.755 kg. I am giving her nanz pro every 3-4 hourly 6 times a day. Shez passing motion twice a day and urine around 10/12 times. Her cold began after catching infectoon from me on 31st October. Since I wasn't comvinved on giving antibiotic, I have just been giving her steam with ajawain. I am avoiding her massage too and just sponging her. I hope I have answered all questions

Hi dear XXXXXXX ..I understand your concerns.

1. As per WHO - World Health Organization recommendations, if bothers milk is inadequate for the baby, the best way to feed is by spoon or paladey as bottle has been found to be associated with future dental caries, teeth malalignment, recurrent respiratory infection, diarrhea, ear infections etc.

2. If you are already feeding her with well sterilized bottles, I suggest you feed the baby after putting saline nasal drops as it will relieve nasal stuffiness before each feed and make the baby feed comfortably.

3. You can use the same formula you are currently using as per dilution instructions on the pack.

4. Your baby's weight gain is appropriate at 25g/day on average.... That's good.

You haven't commented on her feeding. Are 4 ozs per feed ok as per her age n weight every 3/4 hours??

Hi....it is appropriate. But the quantity always depends on the kid's satiety, for a healthy well growing kid like yours....this means that the kid only should decide how much goes in.

Another indirect way of finding out whether the quantity is sufficient or not is by the questions u already asked you..... She's doing perfectly fine.

If a kid is thriving well like this, with appropriate weight gain and with good urine output, it means the quantity is sufficient.

Dear Dr,

My baby is still suffering from congestion and cold and now she has developed mucus in her stool. The stool is not watery but sticky and she is passing air and I feel she has discomfort i passing stool. Kindly advice.

Probably antibiotic associated diarrhea

Detailed Answer:
Hi XXXXXXX ...It seems like an antibiotic associated diarrhoea. Once it starts it will take 5-7 days to completely get better. Unless the kid's having low urine output or very dull or excessively sleepy or blood in motion or green bilious vomiting...you need not worry.

Moxclav is known to cause some amount of diarrhoea antibiotic associated diarrhoea.
I suggest you use zinc supplements (Z&D drops 1ml once daily for 14 days) & ORS (Each small packet mixed in 200ml of potable water and keep giving sip by sip) as hydration is very important and crucial part of treatment.

As there is antibiotic associated diarrhoea, you also need to give a Probiotic like Saccharomyces boulaardii (Enterogermina or Stibs or G-Norm 1/2 XXXXXXX twice a day for 5 days)

If there is vomiting you can use Syrup Ondansetron (as prescribed by your paediatrician/this is a prescription medicine).
