Question: I am 35 years old and I am 13 weeks 2 days pregnant. Baby's heartbeat is good and CRL shows 12 weeks 6 days. dr said I have severe oligohydromnios and baby has to be terminated. Isn't there any cure or remedy? I don't want to lose this baby.
Can we wait for some more time?

Brief Answer:
Liquor cannot be increased.

Detailed Answer:
Hello,
Thanks for trusting us with your health concern.
Severe oligohydramnios means the amount of liquor around the baby is too less and the baby cannot survive without proper amount of liquor. At 13 weeks there already has been a growth retardation of one week.
Even with the best medicines it is not possible for the baby to reach full term and survive. It is best to get the pregnancy terminated rather than waiting till 16-17 weeks and then aborting a dead baby.
Please feel free to discuss further. I will be glad to answer the follow up queries that you have.
Wishing you good health.
Regards.

I have decided and accepted to terminate. Is it fine to have natural miscarriage at home with such low fluid? If yes, what medication would you suggest for the same?

My dr. Wants me admitted to induce labour and terminate.

Kindly suggest - hospital or home, which is better? What complications can occur if at home?

I need to decide immediately, please respond asap.

Brief Answer:
Get admitted for termination.

Detailed Answer:
Hello.
Thanks for writing again.
An abortion at 12-13 weeks cannot be done at home. It needs admission and proper induction to have a complete abortion. At home there can be many complications difficult to manage.
Hope my answer is helpful.

Thankyou for answering to my follow up question mam. Is D&C optional? Do I definitely need to have D&C done? Should it be done After the expelling is complete only?

Answered by Dr. Dr. Rakhi Tayal (7 hours later)
Brief Answer:
Only check curettage is done.

Detailed Answer:
D&C is needed if the complete expulsion of products does not take place with the help of medicines.

Check curettage is usually needed in most of the cases to remove the remnants of placental tissue. This is done after the expulsion of products.

Regards.

Hi doctor,

As suggested by you, I got the baby terminated with induced labour followed by curettage.
We also got the karyotyping test done for the foetus. (I shall upload it)
I am 35 years old, but am desperate for the second baby even after two miscarriages.
Is there a way to know why I had the Subchorionic haemorrhage during this pregnancy? And why there was oligohydramnios?

Can we try again? What's the earliest and latest that I should try to conceive? How can I make sure we don't get into such problems again?

In the 11th week, I had a subchorionic haemorrhage and spotting and light bleeding. I was admitted to the hospital and was on bed rest for a couple of days and after three days the clot seemed to have come out and since then I had brown spotting alone for more than a week. When we thought things were fine now and went for the NT scan, oligohydramnios was detected. After waiting for 3-4 days, I started bleeding again and then went for termination.
Baby was healthy at all times and karyotyping report also seems normal.

I would like to know what could I have done to save my pregnancy. What can I do next to ensure I have a safe and healthy pregnancy.

Brief Answer:
Precautions needed for next pregnancy.

Detailed Answer:
Hello.
Thanks for writing again.
Subchorionic bleeding occurs when the placenta detaches from the original site of implantation. This can happen due to frequent intercourse or low levels of hormones like progesterone. Once the placenta detaches the blood supply to the baby is compromised and lesser amount of liquor is formed around the baby.
To avoid any such recurrences you should give a gap of 6 months before planning the next baby so that all the hormonal levels come back to normal. Do get yourself evaluated for hypertension diabetes or any bleeding disorders. Do take adequate precautions like rest progesterone supplements etc in the next pregnancy after consulting your gynecologist.
Hope my answer is helpful.
Regards.
