Question: Hello Doc ! My TSH level is 7.4 and I take Thyronorm 12.5 . Is the dose correct ?

Brief Answer: Thyronorm dose is correct. Repeat TSH every 8weeks Detailed Answer: Hi friend, Thanks for your query. Firstly, I would like to share with you the following facts:- 1. A TSH value between 5-10 is categorized as Latent (very early stage of under-active thyroid). The general consensus is that treatment should be started when TSH value exceeds 10. The other group of experts are of the opinion that- with the condition being gradually progressive- an early treatment should be initiated with low dose of Thyronorm. 2. I believe that Thyronorm should be started at low dose (12.5 micrograms) even during Latent stage. 3. Hence, I am of the opinion that the dose of 12.5mcg is CORRECT. However, the dose needs to titrated/adjusted, based on the TSH value, done every two months. 4. The dose of Thyronorm should be adjusted so as to maintain TSH around 1.0 or less. You may therefore, continue with 12.5mcg/day and get your TSH test done every 8 weeks, so that the dose can be modified and the target TSH value of 1 is reached. Thanks for the detailed reply. In short till my tsh level is between 5-10 then my dosage is 12.5. However what I dont understand is the target level 1 . Is that what is the ideal level is ?. Sorry for the split qs Dr XXXXXXX Should my Dr keep on increasing my dose till the tsh level tsh level of 1 is achieved.

Brief Answer: Dose is increased till TSH level of 1 is achieved Detailed Answer: Exactly! Your doctor should keep on increasing the dose of Thyronorm till the TSH level of 1 or less is achieved. However, TSH test should only be repeated at least 6-8 weeks AFTER the dose is increased. You may therefore, get TSH test done 6 weeks after starting 12.5 The higher the TSH value, the more is the deficiency of thyroid hormone. Hence, the aim should be to bring down the TSH value AND maintain it around 1.
