Is Tooth Extraction Safe When Suffering From Diabetes?

Question: My A1C is 7.8 and I have to have a tooth extraction. I take Metformin EL 500 mgs. twice a day. Is it safe for me to have this tooth extracted? extraction is needed because the tooth cracked after having been prepped for a crown.

Dear Friend.
Welcome to HCM. I am Dr Anshul Varshney. I would like to tell you that diabetes is as such not a contraindication for tooth extraction. Only problem that can occur is secondary infection after procedure.
Your HbA1c is high.
I would like to know following:
1. What are your fasting/random blood sugar levels?
2. Was Metformin started after HbA1c test or you were taking prior to it?
3. What is your current blood sugar level?
4. Any history of hypertension that is high Blood Pressure?

1. fasting random sugars range between 95-179.
2. I was taking Metformin prior to last A1C testing. Have been on Metformin for 8 yrs.
3. My current blood sugar level at this moment is 173, could be due to tooth ache at this moment, but normally would be about 114-115 at this time.
4. No history of high blood pressure with me. My sibling and mother has it, but thankfully, not me.

Dear Friend.
If your blood sugar level is around 176mg /DL you have to control it before you go for tooth extraction.
If this was a single reading get it tested again. Fasting and Post Prandial with Metformin. I believe it is high because your HbA1c is high even when you are taking Metformin daily. In such conditions I prefer to add Glimiperide 1 mg once a day.
Get your blood sugar rechecked and review with me.
