Question: Is a 4x4 millimeter right hepatic cyst anything signifcant? Or something that can just be ignored?

Hi.
A hepatic cyst is a fairly normal finding. It all depends which area of the liver it is. Also depends upon what problems you are facing like pain, fever, nausea and vomiting. Do let me know what problems you are facing. Also let me know what investigations have been done. If you could send me the reports it would be better and I would then be able to advice you the course of management.

I have been having intermittent shortness of breath, which improves when lying flat. Also a lot of bloating/belching/constipation and pressure/discomfort in chest and epigastric area. No fevers or vomiting, but I do have nausea.

Here is the CT report

CT pelvis:

The uterus is normal in size. The appearance of the endometrial cavity suggestive septate or arcuate uterus. There appears be intact fundal myometrium. No free fluid is present. There is no CT evidence for colitis, diverticulitis, or pelvic abscess. No iliac or inguinal adenopathy is seen. Lumbar vertebrae are normal in height.

Impression:
1. Negative for active pelvic inflammatory process.
2. The appearance of the endometrial cavity suggests arcuate uterus or septate uterus.
3. The enhanced CT pelvis was otherwise unremarkable.

Narrative
CLINICAL HISTORY: Abdominal discomfort. Bloating.

COMPARISON: CT thorax dated 10/30/2015.

TECHNIQUE: Enhanced CT Abdomen and Pelvis was obtained after the administration of oral and IV contrast

The CT was performed using one or more of the following dose reduction techniques:
Automated exposure control,
Adjustment of the mA and /or kV according to patient size,
Use of Iterative reconstruction technique.

FINDINGS:

CT abdomen:

Lung bases are unremarkable. There are no pleural effusions.

The liver and spleen are normal in size. 4 x 4 millimeter right hepatic cyst is present on series 3 image 38. The stomach, pancreas, gallbladder, and adrenal glands are unremarkable. Kidneys are normal in size and appearance.

Aorta is normal in course and caliber. There is no retroperitoneal, retrocrural, or peri-aortic lymphadenopathy. Small umbilical hernia is present containing omental fat only. There is no ascites.

Thanks for the reply.
By this CT description it seems to be a Simple cyst which you need not worry about. I hope you have had a Physician consultation after the CT. If not, then please see your Physician with the CT report and tell your symptoms. He can then rule out an Amoebic or Hydatid cause of the cyst, though the CT would have mentioned if it seemed like any of these. I don't think you need to worry about it.
As for the nausea and other symptoms I suggest you start with Capsule Omeprazole 20mg twice a day half an hour before meals. Take this for two weeks and then you can decrease the dose if it helps your symptoms.
