Question: Is it possible for a male to ejaculate urine into the virgina when he does not ejaculate semen after a TURPS
operation? Thank you


As I felt a great deal of fluid from my partner who does not produce semen
after his prostate op. TURPS last October I believe it was urine (is this possible)?
I have been treated ever since with Antibiotics (Cephalexin) which hasn't cleared
it up & my GP does not know what to do now! Thank you for any advice.

Hi,

It is possible for a male to urinate inside vagina after TURPS, though its rare.This could be because of over burning of urethral sphincter leading to incontinence. Also, please remember one thing. Any male with TURPS can still ejaculate semen. They still have semen but minus prostate component.

Now moving on, rare case if your partner had ejaculate urine I don't find any reason for you to be started on antibiotics. Urine from the vagina shall immediately drain down because of gravity and thus should not cause any infection.

I would like few additional information:
1. Did you have any vaginal infection because of which antibiotics has been started?
2. Did you have any vaginal burningor secretion before this above incident took place?
3. Currently what are your problems and why are you on antibiotics?

I did not have any vaginal infection.
It is an infection in the bladder & I have had my urine tested 3 times & I am
told that I have Escherichia coli & am on my third lot of antibiotics Cephalexin
250 mg. My last ones were 500 mg. The result of my last urine test was
"possible urinary tract infection". As I am 77 it is the law that the nurse must
take a urine sample & this was the beginning last October. I do not have any
symptoms except a restriction of completely emptying my bladder in one go.
I believe Ecoli can cause other problems like kidney failure etc. I had such a gush
of fluid whilst having sex (my partner explained that he never produces semen
when I queried him about the fluid) & it didn't feel like semen but like urine. Soon
after this he had pain & rushed to hospital with kidney stone in the urethea which
recently cannot be found with a scan & he didn't feel that he passed it. Thanks

Greetings of the day
Dear Ms,

Let me explain you normal physiology.
Only erect Penis can enter vagina and can stay inside as long as it is erect. Once the erection is lost after ejaculaltion or climax , its expelled out automatically from vagina. Its not possible to urinate with erect penis. So its unlikley the cause of infection is urine of your partner. If that was the case your partner would have had severe symptoms.
Source of E coli is usually fecal. The most common cause of infection in females is self contamination of urethra from fecal matter/anal contents. The cause for persistence of infection in your cas could be due to
1. Oragnism resistant to antibiotics
2. The organism is highly virulent
3. Post-menopausal state: Its difficult for eradication of infection in post menopausal state because of loss of protective urethral and vaginal environment present when the cycles were occurring and oestrogen was abundant.
4. Contamination of urine sample while collecting: When the urine sample is collected if it touches the surrounding anal region it may get contaninated with E Coli

Do not worry E coli will not cause Kidney failure unless it involves the kidney . As the infection is only restricted to Urinary bladder , so do not worry about it.

I would suggest the following
1. Initiate Antibiotics as per Urine culture and in vitro antibiotic sensitivity report.
2. Long term suppressive Chemoprophylaxis will lead to erradication: Low dose antibiotic taken once daily
