Lump Under Rib Cage, Popping Sensation, Discomfort, Gas, Abdominal Pain

default
Posted on Mon, 17 Sep 2012
Question: Hello
When I push under right side of my rib cage there is a lump that causes a clicking/popping sensation.
It seems like it is attached under the bottom right rib
When breathing or laying on side sometimes the same popping sensation is felt in same position
Causes serious discomfort especially when pressed but not sharp pain
Also feels like there is gas in that area and to the right side of stomach
Appeared after running roughly 2 weeks ago - sharp pain was felt during running in upper right side of abdomen, 2 days after running the lump was first noticed.
No other symptoms.
Thank you

Hi, AAAA

Thanks for your query.

Feeling a lump causing clicking/popping sensation on pressing in the right lower rib cage is most commonly caused by a distended gall bladder. Besides that a cyst, diaphragmatic pathology, a large hydatid cyst, polyp all can cause same problem. For that you need to have atleast a ultrasonography (USG) of upper abdomen including lower part of right hemithorax done for proper evaluation. Many a times a distended stomach may cause the same problem. If USG detects anything further investigations should be done guided by the USG finding. So do consult your doctor immediately and go for USG upper abdomen. Along with the USG i also suggest to go for a chest x-ray (PA view) to detect thoracic pathology and chest wall soft tissue mass in particular. If detected then USG guided minimal invasive procedures like FNAC (fine needle aspiration cytology) may also be done to reach the diagnosis.

So you must go for USG upper abdomen and right hemithorax and chest x-ray PA view to have a priliminary idea of your actual disease. Do consult your general physician and get examined locally on that area.

Dont be afraid. If you have further queries in this respect you may write back to me. You may mail me the investigation reports. I am always here to help and guide you.

God bless you.

Thank you Dr. XXXXXXX Maji.

Your answer was very interesting, however after further self investigation I have found that the small lump seems to be on top of the top abdomen muscle, below or attached to the bottom right rib.

Could the lump be related to a ligament/cartilage or muscle injury? as it seems to have appeared a few days after a sudden sharp pain while running in the upper right abdomen.

The lump does not hurt but causes some discomfort.

Thank you again and I look forward to hearing back from you.

Kind Regards,

Hi AAAAA

Thanks for writing back.

Yes it can arise from anterior abdominal muscle and extends to involve lower rib cage. One certain tumor is there named desmoid tumor which is locally invasive arising from muscle layer and may involve surrounding structures.

So please do an ultrasonographic evaluation of the anterior abdominal wall that will give you clue to the exact location of it and also help you to go for minimally invasive diagnosis like FNAC to fetch the diagnosis. Again it may be a simple myolipoma or other cystic structures.

So you first go for USG which i have first prescribed.
I am vary happy to know that my answer has made you think and propelled you to do self examination. Its good for every patient to do a self examination and self realisation of their disease.

Please keep in touch.

God bless you. Wish you quick recovery.
