Noticed Small Lump Around Thyroid Area. Done Blood Test And Thyroid Test. Reason?
Question: Hello,
1. I have noticed a very small lump around my thyroid area, it is very small and seems movable as when I swallow and place my finger on it, it is not always there but sometimes. I have had blood tests (5 months ago) and it stated normal or euthyroid function levels, although I cant remember if they tester TSH only or T3 and T4 also. I have not had a new blood test recently though.
2. I do feel tired but cant say if this is due to other causes like stress etc.
3. I also experience tight neck muscles (feels like a cramp or spasm) especially the muscles on the right side of my neck just below the ear. My head seems tilted especially when sitting or walking. The pain is a chronic type as lasted over 18 months. At times I do feel a type of chocking/squeezing feeling on the sides of my neck. My jaw muscles especially the right one are tight and at times a little sore.
4. Past few days, occasionally I have felt a bit lightheaded and nauseated, better today.
5. Last GP did blood pressure and from memory 112/85 which I was told was low. My blood pressure last year was more normal i think eg 120/ something.
6. I am in the process of having uterine fibroids investigated as one is 5 cm.
I am worried about my symptoms, what they could mean and if any are related.

Lumps in the neck that move with swallowing are : (1) thyroid gland (2) thyroid isthmus lymph node (3) thyroglossal cyst
If there was any abnormal function of the thyroid gland, it would have shown up with the TSH levels. If the thyroid is not making enough t4 and t3, there is an increase in tsh release.
If the thyroid is making too much t4 and t3 , there is a decrease in tsh.
The thyroid gland develops from the lower part of a duct/passage called a thyroglossal duct. If a portion remains open, it forms what is called a thyroglossal cyst.
The cyst is not cancerous and is removed if it causes symptoms such as issues swallowing or breathing.
(2, 3) If you are presently experiencing stress then it can cause fatigue as well as tightness or tension in the muscles in the neck.
Stress or anxiety is one of most common causes of this tight sensation.
It can also occur due to abnormal movements of the neck , trauma to the neck, ear infections , side effects of some medications or poor posture.In rare cases it can be due to tumors in the skull, this is rare.
This is referred to as torticollis where the head and neck is fixed in an abnormal position.
If the muscles are tensed or contracted ,there would be a pull towards the contracted side.
(4) there are different causes for lightheadness and nausea such as ear infections, meniere), gastroenteritis, systemic illness, stress, side effect of medication).
If the uterine fibroids are associated with significant blood loss it could cause anemia and a lightheaded sensation.
(5) the normal blood pressure is a systolic (top reading ) of less than 120 and a diastolic of less than 80.
120-139 (systolic) is prehypertension or headed towards hypertension
80-89 (diastolic) can also classify a person as prehypertension
112/85 may be low for you if it is normally higher than this but would not be considered low. The diastolic is actually in the prehypertension range.
So the blood pressure reading is a cause of your lightheadness because it is not low but it is possible the fibroids may contribute to this.
Pain and stiffness in the neck can be due to stress but there are other causes listed. You can consider visiting your doctor to determine if any of these may be contributing to your symptoms.
He or she can also determine whether lump is due to any other structure than the thyroid gland.
