Pain In The Right Flank , Shoulder Blades And Right

default
Posted on Tue, 27 Aug 2019
Question: Pain in the right flank , shoulder blades and right side of upper abdomen with excessive belching and flatulence. The shoulder blades and flank pain is more like a constant pressure and dont know if its a referred pain or some muscular issue. Its been there for almost 4 months along with excessive belching. I belching close to around 50-100 times in a day. When I eat something fatty like cheese, oil or spicy food, I belch/burp a lot and feel the pain in the abdomen area.

I also have constant pressure on the shoulder blades and flanks on the right hand side, not sure if thats a referred pain or functional pain. It started after I started using a wedge shapped pillow to raise my head while sleeping.

Is this related to some bile duct issues or something less worrisome as gastritis or Gerd or ibs?

I have attached the image of where the pain is.

I have already referred to a gastro and done couple of tests.

I have done CT scan of abdomen, MRCP, Upper GI Endoscopy and US of abdomen, MRI of Spine = All Normal.

I have done CBC, GGT, Lipase, Amylase, LFT = All Normal.

I have also done Elastase, Candida, H-Pylori, Fat Malabsorption stool tests = All Normal.

The symptoms are still there. If I eat anything fatty or spicy I feel the pain in the region where I have marked in blue. It feels like something is there in the stomach region, not sure if that's the stomach or Pancreas or something else.

My weight is pretty normal and have increased a bit of weight, diet is pretty normal.

Need some help with this, whether I should push for further evaluation ?

Also, what could be the reason for my symptoms?

Brief Answer:
No further test required a case of dyspepsia with gut depleted flora.

Detailed Answer:
Hello and Welcome to 'Ask A Doctor' service.
I have reviewed your query and here is my advice.

As per your complaint, it seems that you are suffering from an excess intestinal gas formation that is leading to the excess formation of intestinal gas. This gas exerts pressure on intestines through which nerve passes causing pain. As many dermatomes overlap each other pain in an area may radiate to other. As CT etc are normal gall bladder pathology is out of question.
Now, let's move to various levels involved in our digestive process which may be the culprit behind it;
Our gastrointestinal tract has a forward propulsive movement (peristalsis) that moves the food material forward as it is digested, absorbed and finally expelled as feces. If somehow this movement gets hampered, the bowel habits would not be proper. This may lead to the formation of colonies of bad bacterias and yeast in your gut. They causes local inflammation in intestines hence digestion is hampered. Taking oily fatty food makes the situation worse.

let me give you a brief idea about what is intestinal flora?
Our body has its own environment called gut flora. Gut flora is the complex community of microorganisms that live in the digestive tracts of humans. A number of factors including age destroy this flora and can lead to various problems with a digestive system like indigestion, abdominal pain, belching, bloating and increased gas formation. I would suggest you start good quality probiotics for a few days
Degradation of undigested food in the intestines by the intestinal bacteria would lead to the excess gas formation.

Treatment-
Tab Rifaximin 550 mg once daily 9 a.m-9 pm if you can convince your G. P For a prescription. Take the print out with you and if he does not prescribe you then you have all rights to question why and what's his line of treatment.

Cap Omeprazole 40 mg once daily in morning empty stomach 6 a.m
Tab Pankreoflat /Semithicone/Activated charcoal twice daily 9 a.m-9 pm over the counter

Cap econorm thrice daily(probiotic) thrice over the counter.
All are easily available take them for 7 days and things would be much better.
If you have any antispasmodic or anti inflammatory with you like ibuprofen take it twice daily. If you don't have it boil 10 leaves of saffron in water and drink it thrice(natural anti inflammatory).
If possible follow this also,
You should also take walk after meals.
-You should take small portions of food every 2 hours.
-Avoid heavy and large fatty meals.
-You should also limit intake of fizzy drinks, smoking, and excess chewing gums.
-You can take Simethicone as it helps in reducing intestinal gas.
-Activated charcoal pills can be helpful.
-Drink sufficient water.
-chew your food slowly and properly.
Hope I was helpful,
Thank you!

Lastly if possible please take vegetarian food for 7 days home cooked with lots of curd.

Follow up after 7 days you would feel much better.
Warm regards.

So with MRCP and CT scan we can safely rule out cancer and other stuff with bile ducts right?

My doctor is asking me to get an EUS done and I am not sure if I want to do it now, may be after trying out some home remedies or medicines.

My diet and weight are pretty normal and so are my blood works, is EUS necessary say after 6 months of trying out different medicines and home remedies?

Also, does the acid irritation causing my stomach pain and pain in the ribs and back?

Thanks

Hello again,

1) Yes with 99.9% accuracy rest 0.1% no test is designed to confirm it.

2) The only purpose of doing EUS right now is to confirm Dyspepsia and acid peptic disorder which you have clinically.

However, to rule out H.pylori and ulcers in that area EUS can be planned.
Kindly confirm if you are in XXXXXXX XXXXXXX or in Western countries.
My further suggestions would depend on that.

Yes, first most common cause of chest and ribs pain in a non-smoker less than 50 years of age is gastritis and acid peptic disorders.

Awaiting your reply.

Hello again,

In that case, initially try lifestyle modification with over the counter medications I suggested earlier.

Remember in starting you have to take medicines along with other changes once the problem is a bit controlled we would start tapering the medications off.

The least we can do is start over the counter meds with diet modification.

H.pylori infection could be ruled out by breath test (non-invasive).
However, if pain continues even after that go for EUS to rule out ulcers.

Give me some time to observe how you respond to the basic medicines.

Hope I was helpful.
Please do feel free to clear any doubt any time.
Follow up after 7 to 10 days if you take the suggested medicines.
Have a fast recovery

One last question, so based on my symptoms and evaluations there is nothing like urgent in nature that needs to be investigated right?

I have done stool test for H-Pylori, is that not enough? I spoke to my doctor about the breath test, he was like the stool test is enough and that my symptoms are more or less inclined toward gastritis/ GERD.

EUS is just one last step to rule out anything that the MRI and CT scan cannot see.

I am not sure what to do.

Thanks for hearing me out

Detailed Answer:
Hello again my dear,
Our gut in lay man's language is divided in two parts
1)Upper- All tests have been done. No chances of tumour or any mass etc.
2)Lower- None of the test covers it. A colonoscopy can be done. Once it's normal you can get assured that there is no growth, tumour or mass in your complete gut from mouth to anus.

Sir as far as MRI and CT scan is concerned they have no role if we suspect a patient to have gastritis or ulcers. Gold standard test for suspected case of gastritis or acid peptic disorder is endoscopy. MRI uses magnetic force almost 3 Tesla to break water particle inside every organ. Any organ having an extra mass or out growth or tumour would need extra blood supply to that area because tumour is also a cell and needs nutrients. So when MRI would be done the part having tumour would reflect extra amount of water as compared to other parts.
Now in acid peptic disorder or H.pylori infection or gastric ulcer there is no increase in blood flow to that area so simply MRI is not meant for that.
In endoscopy a high definition camera is put on top of a flexible tube and is inserted into your upper gut via mouth. Any growth ulcer inflammation (gastritis) could be seen directly by camera.
Same thing is done in colonoscopy but through anal canal.
You are free to follow up any time.
Hope you understood the concept and significance of each test.
Yes if stool test is ok then don't worry for H. pylori though again gold standard is endoscopy only.

If you close the thread please do suggest how to make answers better in case you have any suggestions.

Thank you and it's not an emergency just follow medicines and proper diet and enjoy life.

Thanks a lot doctor for the detailed explanation. Is it common to see people with symptoms similar to mine? Like chronic pain in the abdomen due to acidity? And does it go away or cured? Or is it a lifelong thing where I need to take care of my diet and lifestyle?

Hi XXXXXXX
In non smokers and non alcoholic almost every 9 patient out of 10 patients presenting with chest pain and all are having severe gastritis. Heart is an overhyped issue and age for heart problem in male is after 50years.
Yes it's very common.
Yes it can be manged very easily but for that you need proper medicines which Americans are too skeptical about. If you have some one in XXXXXXX get 21 days medicines initially and you would be 95%bwtter in just 3 weeks. Right now the biggest problem is that all I can do is to tell you 2,3 over the counter medication which are not even designed to treat proper gastritis and reflux problem.
Life style modification is required but be practical. Tell me how Long can you eat bland food?
Through out life?
Is it practical?
And all antacids are very safe. Even if you will get any minor side effect that would take minimum 30years.
The region marked in blue area is called Epigastric region. That is where your stomach lies. Your stomach has inflammation. In simple language if you press a sports wound in your skin would you feel pain or not?

ANSWER is yes.
In the same way your stomach is inflamed and wounded and Americans and Canadians won't give you proper medicines.

Genetics has a role in every thing but believe me i need just 21 days to provide you relief. Even i can ask my son to post you a prescription in XXXXXXX but you need some one who can buy those mere and can post you.

Hope you can understand life style modification would help but initially we have to comfort your gut by giving medication.

My parents are coming to US next month, I can ask them to get the medicines.

I know you have prescribed some medicines, can you state them again, so I can ask them to get it when they come over?

I am planning to go on a FODMAP diet, my doctor has asked me to go on a FODMAP diet for 2 months along with a course of antibiotic Xiaxfin (not sure if I got the name right).

Will this diet be enough? I can go on a bland diet, its much easier to follow this kind of diet vs doing it in XXXXXXX

One more question, why did the doctor not diagnose me with gastritis after an endoscopy?

Hi again dear,
Tab Nexium 40 mg twice daily.
Tab Domstal DT once daily
Syrup Mucaine gel two table spoon three times a day
Tab Acotiamide 100 mg three times a day.
Tab pankreoflat thrice daily
All are common brands of XXXXXXX and are available freely.
Ask them to bring it for 1 month.

Very likely stress is the main cause. When you are outside your town your anxiety levels are low. Anxiety stimulates autonomous nervous system and stimulates vagus nerves which causes surge of acid in stomach hence causing symptoms.
Take lactium an anti anxiety cow milk derived supplement daily.
kindly mark you anxiety levels when you are at work on a scale of 0 to 10.
How's your sleep cycle?
What type of personality are you-
1) Satisfied by what you have
2)Perfectionist and always craving for more.

You can follow any fancy diet like Fodmap or vegan or detox. But from Allopathic(English point of view) my only concern is food should not have spices and no alcohol and smoking.

Take a big bowl of 4 types of fruits in morning and a bowl of 4 colored sauted vegetables in evening.

Gastritis means inflammation and is directly related to food. Mild to moerate gastritis can be missed in gastroscopy and in a normal thing.Patients are treated symptomatically.

Pain In The Right Flank , Shoulder Blades And Right

Pain in the right flank , shoulder blades and right side of upper abdomen with excessive belching and flatulence.
