Pregnant, Can't Keep Down Anything In Stomach In Hyperemesis Gravidarum. What To Do?
Question: if a lady can,t keep downanything in stomach in hyperemesis gravidarum. How to manage the situation?

Some amount of nausea and vomiting are common in pregnancy.
If woman presents with hyperemesis gravidarum, the measures to be followed are :
-Oral antiemetics with combination of doxalamine and pyridoxine are approved to use in pregnancy.
-The lost fluids and electrolytes should be replaced in small quantities at frequent intervals.
-If patient not able to take anything orally, intravenous infusion should be given.
Among the intravenous fluids, half of the amount should be 5% dextrose.
-Intravenous antacids and antiemetics can be given.
-Once the vomitings get controlled oral fluids can be started.
-The cause of the hyperemesis to be investigated. It can be seen in conditions like twin gestation, molar pregnancy etc . So these conditions should be ruled out.
-Presence of urinary ketonebodies to be investigated to estimate the severity of the condition.

Better to avoid especially in late pregnancy, because it can affect the baby by its prostaglandin inhibiting effect.
