Semen Analysis Showed Motility 46%(low) And Sperm Count-5 Million(low. Does This Mean I Am Infertile?

I recently completed a semen analysis.
volume - 2.5mL
**Liquefaction - 60min
viscosity - normal
**motililty 46%(low)
**sperm count - 5 million (low)
appearance - normal
vitality - 85%
pH 8.5
**%Abnormal - 52%
Notes: variation in size of heads/tails.malformation of acrosmal/postacrossomal region. An increased number of abnormal forms are noted, generally assumed that such abherrant speramtozoa are incapable of fertilization.
Does this mean I am infertile?
My wife was at her peak 2 days ago. To help we used preseed gel and a cup.

I'd been working out with an Insanity program over the past 3 months with no issues, taking Spurotein protein shakes..haven't done so since this episode 2 weeks ago.
bloodwork
initial Creatinine of 127, eGFR of 55, UREA 8.0, CK 206
a day later: Creatinine 110, eGFR 65, UREA 7.4, CK 147
Blood work a week later: Creatinine 109, eGFR 66, lipids, normal, thyroid normal, sodium, potassium, cholesteral within normal limits.
I'm going for more blood work tomorrow for pancreas, kidney, + ultrasound on stomach.
Symptoms over past 2 weeks: at times feel bloated/tight ribs, low to mid back after eating. Eating and drinking water - belching more than normal.
UNABLE to fall asleep...try to get to bed for 11pm, unable to fall asleep until 3or4 and up at 7ish...more difficult to breathe if it's damp/humid/warm.
Sometimes still feel constipated, bloaty, lack of energy, warm sensations/light headed. At times I feel I'm not getting enough O2 to lungs have to walk around...can't lie down on my stomach..sleep (try to) more upright...also noticed i'm becoming sensitive to sounds.
Any ideas on what it could be?
Thanks for your query.
I have read your query & I understand your concerns.
Following is my reply:
1) Your sperm count is very low.
2) I advice you repeat the semen analysis test to confirm if count is surely that low. If you get almost same low count again, the better way to try for conception by ICSI ( Intracytoplasmic sperm injection ) technique where your sperms are injected into your wife's egg after taking them outside the body.
3) This procedure is done in any good IVF centre in your place.
This is the reply regarding first part of your question about fertility.
The second part of you question regarding medical part will be answered by a medical expert and hence I request you to ask second part of the question separately again as it will go to a specialist in medical field.
I hope I answered your query regarding fertility. I will be available for any followup queries you have.
