Shortness Of Breath With Pain And Pressure On Chest. ECG Normal. Do I Need An Angiogram?
Question: F, 32 yrs, Mother of 3 year old, severe chest pressure in the center and poking pain when taking XXXXXXX breath, tightness as if someone is squeezing/heaviness on chest, discomfort, shortness of breath. ECG/ECHO/TMT done today, all came out clean. Still unable to take full breath. Pain shifted from center to upper left chest towards shoulder blade. Opinion? Do nyou suggest we go in for angiogram?

A negative TMT practically rules out Coronary Artery Disease.
And this is not common in women of this age.
If in doubt, the next step is TMT with thallium isotope. It is the ideal non-invasive way to evaluate ischemia / to assess the PHYSIOLOGY (function) – to see whether the blood arriving at the heart muscle.
If there is a suggestion, the next step is to see the ANATOMY (structure) – CT angio is non-invasive study for the anatomy.
Angiogram is the gold standard, but invasive
Chest and spine X-rays (CT scan if necessary) may help.
