Question: Dear Doctor

I HAVE SUFFERRED DEPRESSION LAST 15 YRS . CHANGED DIFFERENT DOCTERS BECAUSE OF INADEQUATE OR WRONG TREATMENT. I SUFFERRED FROM DEPRESSION BUT TREATED AS SOME PERIOD AS BIPOLAR DISORDER ,SOME PERIOD AS SCZOPHERNIA CASE.
LAST 6 MONTHS I HAVE IMPROVED A LOT.
I AM ABLE TO REMEMBER,ANALYSE,MY THOUGHT PROCESS IS SEEMS NORMAL.FREQUENCY OF SEX MONTHLY 2 OR 3 TIMES MAXIMUM.
THE SAME QUESTION I ASKED TO DR.. ABHISEK XXXXXXX I WANT TO TO REVIEW WITH DIFFERENT DOCTER.

The folowing pattern suits me to take medicines,
Please review it.
Morning 7am Pexidep Cr / XXXXXXX Cr 25mg for depression
Morning 12 pm Bupron Sr 150mg for smoking & depression
Afternoon 3pm Pexdep Cr / XXXXXXX cr 12.5 mg for depression
Nite 7pm Bupron 150 mg for smoking & depression
nite 10.30 Pexdep Cr / XXXXXXX Cr 12.5mg with Aricep 10mg with multivitamin Revital

I am taking these medicines since last 5 months. Pl. review it & advice. I am feeling ok

Detailed Answer:
Hello,

When someone is feeling better and able to do normal routine works comfortably then meds are usually correct.

However, your diagnosis dosent support the medicine pattern in few points -

1. bupropion is best taken in morning or afternoon hours and avoided in evening hours or later. So, it is ideally taken as 300mg in morning.
It is an exrended release preparation which has its blood levels maintained all day long so there is no need to take it two times a day. One time dosing is sufficient.

2. You are taking aricept, a drug for alzhimers. The query dosent mention anykind of memory or other neurological issue, so, I dont know how relevant it is? Does your brain can suggest something of that sort (or any other reason behind it?)

3. Paroxetine is also a controlled release preparation (CR) form so blood levels are maintained all day long and there is hardly any need to take this thrice a day but if that suits you then there is no harm in doing so.

You may discuss aricept and bupropion dosages with your doctor when you meet him next time.
