Sore Throat, Fever, Night Sweats, All Reports Normal. Cause?
Has undergone several tests over the last 2 years, but nothing has been wrong. Tests include complete blood, spleen, x-rays of chest, stomach and pelvic. Also tested for malaria and TB and scores of other tests for bugs. Several other tests as well. He is now planning to have his case reviewed at Mayo Clinic.

1. Repeated sore throat every few weeks is a sign of a sensitive throat or decreased immunity levels. He needs a throat examination. If there are signs of chronic tonsillitis or lymphoid hypertrophy, this can be treated. If his throat appears normal, a search for causes of lowered immunity must be done.
2. Since his Night sweats are severe and have been present over many years without any other symptoms, Tuberculosis may not be the cause. Does he have any weight loss ? Atypical Mycobacteria are related to TB and may be zoonotic (animal origin). Does he have any pets or has he been to any Aquatic Show? Acute Follicular Tonsillitis or pus anywhere in the body may cause night sweats. Hormonal imbalances, hormone secreting tumors, autonomic neuropathy may also exist. (since he has a history of bronchospasm). Does he have any diarrhea or constipation?
3. Sputum examination for fungal infections (since he has a history of asthma) should be done. Get his Vit B12 and Calcium levels done, preferably a comprehensive metabolic and hormonal profile. Get his 24-hour urine samples tested for abnormalities. Get a GTT (Glucose Tolerance Test) done.Have tests done to rule out chronic infections such as Sarcoidosis, Brucellosis, Lyme Disease and HIV.
4. Obstructive Sleep Apnea Syndrome (OSAS) also gives rise to night sweats. What is his weight? Does he snore? Drug induced or withdrawal symptoms can also be the cause. Is he on any medication currently? Does he have any acid reflux?
