1. "Stress test is positive for ischemia" means, that on treadmill testing (exercise ECG) your ECG showed abnormal ST depression. This infer that a part of your heart is not having adequate blood supply when you were exercising on treadmill. However this comes with a rider, Not all tests shown to be positive by exercise ECG stress testing are actually positive. The test has a specificity of ~76%. Hence it may not tell the truth in nearly 1/4th of a mixed population. We consider many other things while interpreting Stress tests like total exercise capacity, occurrence of symptoms during the stress test, type of ST changes during the test, hemodynamic changes during the test etc. All these help us to ascertain the reliability of the ECG changes.
The best person to decide this is your cardiologist. I would recommend you to see your cardiologist and let him decide if you require any further work up or not. Moreover, he will also decide if you require further non-invasive testing or direct coronary angiography.
