Question: have friend with stage 4 liver cancer. blood pressure 90/40. really swollen and orange. doesn't want to eat. not making rational sentences. drifts in and out of sleep. is the end near.

All the symptoms you have described suggest a liver failure, with hepatic encephalopathy. Unfortunately, the prognosis of patients with acute liver failure resulting from stage 4 liver cancer is dismal and the end is near for your friend.
