Brief Answer: Functional Bowel Disorder [GRAHANI] Detailed Answer: Dear XXXXXXX Thanks for contacting Ayurveda with your health concern and going by your medical condition, I have come up with following Documented evidences. 1. going by your complaints: . since you took medicine(s) for IBS, do let me know whether it was diagnosed by Physician or by a Gastro enterolgist? . were you been evaluated by a General Surgeon for any Anal/rectal pathology [as routine for any sphincter problem].. since history of 'sometimes little leak' is there . as your Lipid Profile is also not good, are you taking any medicines for the same? . as symptoms increases/worsens during time of stress: thus probably could be Hypersensitivity of Intestine [Functional Bowel Disorder] 2. Please revert back with above questionnaire, so that Management aspect can be initiated, in the mean time Preventive aspect is imparted: Take: old brown rice, moong/masoor/arhar dal, banana, jamun, butter milk, goat milk, sesame oil, honey, soup, cumin, mint, fasting. Avoid: newly harvested rice, peas, garlic, vinegar, milk, jaggery, citrus food, spices, night time awakening, drinking excess water, suppression of natural urges, excess physical exertion, walking during peak hours in summer. 3. make butter milk [lassi], everyday and drink . make sure it is made/prepared fresh everyday, do not add water, add fried cumin seeds and rock salt [quantity sufficient] 4. fried cumin seeds 500mg fried asafoetida [hinga] 250mg panchamrit parpati 250mg.. take with honey, thrice a day . soak 5 gm amla in water, when become soft, crush them and add black salt [quantity sufficient], and make lozenges of 125 mg and suck twice daily. PS. going through your reports, main concern is raised Lipid level and there is slight increase in GGT and SGPT [in liver function tests section], as these both are more specific for heapto cellular disorder, but since are mildly increased, talk with your doctor regarding the same. thanks Doctor for prompt and detailed response. Appreciate it. Please see my response below. 1) I was not told that it is IBS or Crohn's or some other disorder but was told that I am given medication to manage bowel. this was done by Gastroenterologist 2) Gastro had one physically manually examined with finger and said sphincter is ok. had suggested doing some pelvic exercise to strengthen. Should I get it examined again and by whom? Surgeon or Gastro or someone else ? 3) I am not taking medicine for lipids. One doctor that I consulted with, suggested that I try without medicine first,. I have started doing 30 minute brisk walk daily in last 15 days and have lost 1.5kg weight. Should I take ayurvedic or allopathic medicine for the same? 4) I Have also at times seen while color seed like substance in the stool. Is that a problem ?

Brief Answer: Lifestyle Management & Preventive Aspect. Detailed Answer: Dear XXXXXXX Thanks for reverting back 1. Since you were not told about any disorder of Intestine, and most Importantly you were examined by a specialist, thus he must have given due consideration in taking up your case, thus do let me know about your lifestyle: . people who drive for long, work long hours or shifts, often have uneven eating patterns which may contribute to variable bowel movement.?? PS. getting into car, or sitting hunched at a desk directly after meals can cause cramped muscle and indigestion. . some people eat nothing in the early part of the day and present their digestive system with an enormous meal later. [which means they have to rely on one meal for all their nutritional requirement]? . Most people have the urge to smoke directly after a meal, which affects absorption, especially of vitamins, and can cause griping and may be a factor in development of motility disorder?? [any addiction of tobacco in any form] PS. Coffee helps to increase movement when taken after meal, but may be too much for a [sensitive] bowel. 2. Since digital examination is done by him, but second opinion is always welcome [thus do consult a general surgeon [proctologist] 3. Since Diet and Exercise goes hand in hand thus following points are worth mentioning [prior to start any medicines] A. Dietary and lifestyle Changes . Tips on Eating: Starvation is not the right way to loose weight. Take the right type of diet and eat heartily: AVOID: .sedentary lifestyle, day time sleeping after meals, newly harvested rice, curd, butter, ghee, cheese, cream, chocolates, cakes, pastries, ice-creams, XXXXXXX . oils should be reduced to minimum, avoid deep fried foods like puries, bhujees, potato chips. .avoid potatoes, sweet potatoes, beets, yam, nuts, fruits with sweet pulp like mango, chickoos, bananas and grapes. . Avoid processed and tinned foods, opt for fresh foods . Drinking cold water after exercise. . Eliminate all cold food and drink; you should eat nothing that is refrigerator cold. WHAT TO EAT: .start the meal with large bowl of thin soup, followed by good volume of salads prepared with green vegetables (cabbage, cauliflower, lettuce, cucumber, sprouted pulses). This will prevent you from taking extra servings of the meals. Drink plenty of water and diluted buttermilk. .Avoid rice, take roti or phulka, without oil or ghee. The bhaji servings should be prepared from green leafy vegetables, onions, cauliflower’s,lady’s finger, radish, turnips, mushrooms, sprouted pulses. Oil may be substituted by gravy of tomato, onion, garlic, and ginger, in the curry or bhaji for taste. . For morning and evening snacks, take recommended fruits, vegetables and low fat (skimmed) milk, black tea without sugar has negligible calories. . Carbohydrates: fat requires carbohydrates to burn, so take them in moderation like grains. . DEVELOP HEALTHY EATING HABITS: . Do not eat only 2 large meals a day. It leads to overeating due to hunger. Take snacks in between. It is advisable to eat a good breakfast and light lunch and dinner. . Keep a quiet and pleasant atmosphere in the dining hall. Watching TV while eating, talking, arguments, angry discussions all lead to overeating. . Eat slowly; chewing each bite thoroughly gives a feeling of satiation. . Do not be tempted to finish up the leftovers. . Use nonstick cooking ware for cooking. 4. going by history of 'seed' like, could be worm infestation [if proved], you can have the stool tested for ova (eggs) and parasites in a stool test. PS. presence of worms can be one cause of irregular bowel, or could be undigested food [since white stool is not there] 5. Prescription medicines which you can initially take is/are: . Kutja arishta & Jeerakadya arishta . Chitrakadi Vati. . Tablet Amoebica. . Bilagyl Lehya . Cruminill syrup. Dr. Munish Naimittika
