Suggest Dosage For Sertindole

Question: Hi,
I was until some weeks ago (three weeks) on Sertindole 8 mg, at the beginning with 4 mg and then 8 mg. There was a clear positive effect with Sertindole (Serlect) in the third week (4 mg, 8 mg), but it didn't happened with another 4 mg (12 mg), i mean there was no effect in the third week and effect of 12 mg is very slow. How much should i wait to see the full effect of this new 4 mg (from 8 mg to 12 mg)? I am also intermediate metabolizer for cyp2c19 and cyp2d6

I would wait 8 weeks from the dose increase (8mg to 12mg) to judge for full positive effect. It can sometimes take this long for the receptors in the brain to stabilize and start functioning fully. If you do not see a positive effect after 8 weeks from the last dose increase, I would consider decreasing back to 8mg, as this would avoid side effects of the higher dose while still maintaining the efficacy at 8mg you describe from before.

Risperdal can be an alternative to go faster in the fight against overactivity of dopaminergic receptors in the brain? Problems with risperdal for weight gain are serious or not?

Risperdal is more commonly used in the USA compared to sertindole, which is rarely used due to its negative side effects. Risperdal does have weight gain associated with it, approximately 10 pounds on average, but has less likelihood of EPS and dystonic reactions compared to sertindole. It is equal in efficacy to dopamine blockage compared to sertindole.
