Brief Answer:
Add Sitagliptin.

Detailed Answer:
Welcome To HCM. I am Dr Anshul Varshney. I understand your concern.
With the course of time your beta cells of pancreas that secrete insulin exhausts and your blood sugar increases.
After studying the details of your blood sugar and HbA1c levels my recommendations would be to add on a DD4 Inhibitor like Sitagliptin in your regimen.
I am very hopeful that once added you would get the desired blood sugar control.
I would also advice you to continue walk as well as strict diabetic diet.
