Question: Hi,my friend had first tonic clonic seizure 3days ago. He had Severe TBI 2years ago and was on Keppra 1250 mg BID for 1year. So, now they started Keppra 1250 BID again. I remember how he was at that time: weak with severe cognitive impairment, drowsy like a drunk. Should it be started with lower doze like 500mg BID?

Brief Answer:
Another anti-epileptic drug should be preferred.

Detailed Answer:
Hi,

Thank you for posting your query.

I have noted your friend's symptoms.

The most likely diagnosis in your friend is post-traumatic seizure disorder or epilepsy.

He would need anti epileptic drugs (AED) for long duration. Since he already had a reaction and adverse effects with Keppra (levetiracetam) earlier, it should be avoided. Giving a lower dose of Keppra may not offer him adequate protection against the seizures.

So, I would prefer suing another AED such as oxcarbazepine or sodium valproate.

I hope it helps.

Best wishes,
