Question: I have a blind / blurred vision spot in my left eye. On a Amsler chart it is two squares to the left of center line with the bottom of the spot touching the center line. Spot is symmetrically round. I can also see the spot when I blink my eye, looks similar to flashbulb spot.

Brief Answer:
scotoma consult, refer to ER to role out stroke

Detailed Answer:
Dear XXXXXXX XXXX,

Thank you for trusting us at HCM with your question.

This is a scotoma, blind spot. Causes are either retinal in nature; meaning a direct injury to the eye internal layer leads to blind spot at the point, or a small bleeding at that layer can lead to this.

Another possibility is that it results from stroke to the brain occipital lobe responsible for vision. In case of having the scotoma in the left visual field that means a lesion in the right occipital cortex.

Therefore, It is important to visit an ophthalmologist and do an eye examination, slit lamp, to look closely at the front and back of your eye.

Also very important is to role out stroke you need to refer to a neurologist, who can order a MRI to role out stroke. Since it seems you were already diagnosed with elevated blood pressure and you are being treated for it, it is paramount that you address the reason of scotoma development as this can be a sign of more serious problems. As hypertension is a risk factor for developing stroke.

Please do not hesitate in referring to your treating doctors, if possible get in touch with ER as soon as possible.

Please feel free to ask me more questions.

Kind regards,
Dr. Nazzal
