I am not taking any antihistamines. I have put on the permetherin Cream as needed. I have taken two doses of oral ivermectin. one one week and another the next. 200 mcg/kg wt. this helped some but it did not clear it out.

The medicines are fine.
The itching in lice infection is due to a hypersensitivity reaction and can take little longer to resolve even after the lice are eradicated.
Even permethrin lotion has to be reapplied after 10 days for complete cure of infection.
I would recommend you to take antihistaminic like tablet Cetrizine 10 mg once or twice daily depending on the intensity of itching for another 10-14 days.

How about the oral ivermectin? What dose do you recommend and for how long?
