Question: Dear Doctor-

Sorry I am posting this question back with more info and I can reply back to the previous question on this forum as the question says closed.

Since 15 days I had problem listening properly with my left ear and had pulsating sounds. I visited a doctor 2 weeks ago and he has examined my ears, ear drums and they were fine and there was fluid behind my left ear drum. He said that the tube connecting the left ear with the throat is blocked and hence because of that the fluid, I am not able to hear properly. I was put on (1) fluticasone furoate nasal spray twice daily (2) Ambroxol capsule (3) Cetzine A tablets. I have been religiously using the medicines. I was asked to undergo some audiometry, Vitamin D3 tests and Glucose Challenge Test before the revisit after 2 weeks.

On a family person’s advice, I went my regular doctor (exactly one week earlier In-fact I know this doctor and went to him on two different occasions but he was not available when I had the current issue initially and hence could not visit him) and he told me that the left ear drum had appeared retracted and there was mild retraction on the right ear drum. He prescribed (1) montelukast tabs (2) fluticasone nasal spray (3) xylomist nasal drops and (4) methylprednisolone tablets to be used twice daily for a week and slowly tapered down.

Now, Since I started using the first doctor’s prescription for last two weeks, I revisited the first doctor with the reports, I would like to share other reports. please let me know how I can share the PTA/IA/SA reports.

1.     Vitamin Serum D3:
Result 10.10 ng/ml

2.     GCT (50 grams of glucose load) Plasma glucose 1 hour :
Result 150 mg/dL

He advised me that since the initial medication is not helping to open the Eustachian tube, he may have to remove the fluid from behind the ear drum and arrange some tubes in the ear. Before that, he wanted me to use few more medicines (Prednisolone and clarithromycin tablets) and come back after a 10 days and see if that helped.

I am really concerned and I do not want to have my ear drum punctured. I had exactly similar issue on multiple occasions before and it used to heal off in a week or two, but this time I am really concerned why it is taking lot of time. I am barely able to sleep because of the anxiety arising out of my current situation. Any better nasal spray to unblock the tube?

You answer certainly helped ease my anxiety. Just to update you after using prednisolone today I feel some relief. I am supposed to use prednisolone tabs thrice/day for 5 days and then switch to twice/day for 5 days.
Any specific difference between using Prednisolone Vs Methyleprednisolone.
The second doctor I think prescribed me Methyleprednisolone and the presceiption he gave was to use it for twice/daily for 2 weeks and one/daily for 2 weeks.

After using prednisolone today, I feel that the block sensation has appears to have improved and even my left ear appears to have opened up a bit. What exactly has happened. Any way, Thank you again for your reply and in the future if I would like to reply back to this query, please do let me know if you can keep this open for few more days.
