Suggest Remedies For Lack Of Ejaculation During Intercourse

Question: During intercourse with my wife,my penis do not ejeculate.I am facing this problem from last 2 days only.Before that it was happened 5 or 6 months ago but after one day it was normal again.But this time it has been 3 days and i tried by masturbation but still the problem is same.
Although my penis is getting hard and I am able to perform sex for 1 hour with my wife but this ejeculation not happening causing me stress.
I am also takng gastro related medicine and DOM-Dt for relaxation given by gasterologist.
Pl Advise me regarding the issue.

Thanks for writing to us. I can understand your concern.

Dom-dt is frequently used medicine and it does not cause such problem.

If there is no orgasm and no ejaculation, it is anorgasmia.

If there is orgasm but no ejaculation, it is anejaculation. Anejaculation is the inability to ejaculate semen despite stimulation of the penis by intercourse or masturbation. The causes can be psychological or physical and anejaculation can be situational or total. In young age it can be due to ejaculatory duct or seminal vesicle obstruction/inflammation.
In most of the cases of anejaculation there is retrograde ejaculation.

Common causes are:

- Diabetes

- High blood pressure and medicines for blood pressure (Phenoxybenzamine, thiazides, clonidine, alpha-methyl dopa)

- Medicines for psychiatric problems ( Paroxitene, fluoxetine, sertaline, thioridazine, chlorpromazine, haldoperidol, amitryptyline)

- Other medicines (Prostate pills-Alpha-blockers, imipramine; Anti-Inflammatories-Naproxen; Muscle relaxants- Baclofen; Other- Alcohol, Methadone, phentolamine, guanethidine sulfate)

- Any problem of urinary bladder or prostate

- Any surgery of urinary bladder , prostate, urethra or spine

- Spinal cord/column injury/problem

One should go for urine analysis to check for sperms in urine. If it is absent ,it rules out retrograde ejaculation.

Scrotal physical examination and sonography would help to diagnose the cause.

- Please let me know is there orgasm or not?

- Is there whitish urine after sex?

- Are you using any other medicine?

- Do you have any physical problem/illness?

- Do you have any stress?

- Is there any swelling in scrotal region or do you feel pain there?
