Yes, it's expected in acute dengue fever to have the thrombocytopenia (fall in platelet count below normal range)..it's only 2 days as per your description, so the acute episode is expected for at least another 48-72hrs (exact time duration varies from person to person, depending on the severity of the infection and individual immunity), after that it'll start stabilizing..

Right now, most important is daily monitoring of platelets and the hematocrits (preferably the daily CBC).. any count less than 50000 should be kept under strict observation..

If any bleeding manifestations (regardless of the platelet count), i.e.extensive red rashes over skin, reddish color urine, black/tarry motion, bleeding from nose, gum-bleed, sub-conjunctival bleeding, along with any difficulty breathing, decreased urine out-put, excessive loose motion, drowsiness, severe pain abdomen experienced, then immediate hospitalization is advised..

Antibiotics have no role in this unless there's any secondary bacterial infection involved..

Papaya juice/extracts are all part of natural remedies, and there's still no proven study reports available (studies are ongoing)..

Continue with the ORS and also drink a lot of water(strictly Mineral/ U.V.R.O purified) ~ 3-3.5 ltr per day..stay well hydrated..

If fever persists, continue PARACETAMOL 650mg Three times daily after food for next 3-4 days..

Have lot of fresh fruit juice, simple home cooked meals, avoid outside junk/deep fried/ spicy foods strictly till this phase gets passed..

It's better to include antacid/PPI eg. Cap PANTOPRAZOLE 40mg Once Daily 15-20min before breakfast for next 1 week..

Avoid using tooth-brush for the time being and use Chlorhexidine mouth wash twice daily instead..
