Suggest Remedies For Overactive And Enlarged Bladder
my husband has a stretched enlarged bladder from driving a bus for many years...
this resulted in bph.
he is on 3 bph meds and his prostate has shrunk quite a bit...now small according to the urologist
however he has recently developed the problem of overflow of urine
he used to have bladder retention...almost never fully emptied his bladder
He has been on a trial of Mybetriq..
the sudden urgency problem seems to be gone
but he doesn't really feel the need til it is almost too late...he is even using Depends now
do you think Detrol LA might be a better medicine for him...or do you have another suggestion

Based on the facts that you have posted your husband who is known to have enlarged prostate since years has developed problem of over flow of urine .
This is mostly due to what is called as Chronic Retention of Urine with over Flow incontinence .It is a common situation in patients having enlarged prostate for many years who develop weakness and loss of tone of detrusor muscles of bladder and land up in a problem of Chronic retention of urine with over flow .
Consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Cystometry study of bladder
In a given situation it is unlikely that any drug would help him to regain his bladder tone .I would suggest to put Suprapubic self retaining catheter to have continuous drainage of bladder .This may need to be kept for months till he regains his bladder tone and able to feel the sensation of bladder distention .

I want to let you know he has good kidney and liver function via his blood work
And has regular urologist visits
And has hadnormal ultrasounds every year for the 3 years he has been dealing with this problem
And has never had an abnormal urinalysis
I.e.
A bladder infection
Chronic retention of urine develops in long standing Prostate Enlargement

Chronic retention of urine is a sequel of Bladder Outlet Obstruction due to enlarged prostate that develops in elderly patient with non compliant bladder in spite of having normal kidney function .
Following tests are the important tests to confirm the diagnosis .
1)Cystometry and Urodynamic study of the bladder
2) Uroflowmetry.
3) Endoscopic Examination of bladder (Cystoscopy).
